Service Description:

Program assists people living with dementia and their families make informed choices for better quality of life
Services may include:- assistance with identifying and accessing information, supports and services that meets needs
- facilitates connections to education, programs and services within the organization as well as other health care and community service providers
- one-on-one counselling
- caregiver support groups
- support groups for persons newly diagnosed
- referral to community resource by phone, drop in, or home visit
