Service Description: Community-based program facilitates information, support, and advocacy for those who wish to remain in their homes for as long as possible, and to ease the burden of care supplied by informal caregivers such as family members while ensuring cultural appropriateness. 

Services are interconnected with other community-based programs and may include referrals.

  • workshops and community events
  • care-giver support 
  • indigenous support service
  • medical transportation and security checks
  • possible help with the acquisition of Medical Assistive Devices
  • supports accessing community resources
