Service Description: Supports for people living with prediabetes, diabetes or type 1 diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff

Additional services available in group and individual settings include:
  • starting insulin
  • insulin pumps
  • education programs
  • pregnancy and diabetes
  • education awareness events
  • education and training for health care professionals
  • community service group

Application: Central referral from Adult Diabetes Central Referral Program * physician referral * health worker referral * self-referral
Eligibility / Target Population: Adults 18 years and older with pre-diabetes, type 1 diabetes and type 2 diabetes
