Service Description:

In-home visiting facilitated by trained volunteers who address the social, emotional, spiritual and practical needs of both people with a life-limiting illness and their family members.
Services and support available:- caregiver support
- support with legacy work or activities
- social and emotional companionship
- working collaboratively on hobbies and interests

Call Community Care Coordinator for an assessment of needs and requirements * Home visit will be scheduled to further discuss the program and to determine the best volunteer match for in-home visiting
