Service Description:

Palliative care residence and services * virtual support may be available
Services available:- in-home visiting volunteer support
- trained volunteers and professional care as well as support to people living with life-threatening illness and their caregiver(s) and to the bereaved
- individual and group bereavement support
- programs to support wellness and psycho-social needs
Campbell House - 10-bed palliative care residence with beds allocated to end-of-life support as well as short stay, supportive palliative respite and pain and symptom management

Referrals made through Home Care Coordinator for Campbell House * self-referrals or made by other health care and support organizations

Eligibility / Target Population:
|
People suffering from a life-limiting illness with prognosis of three months or less * must have involvement with home community care services
|
