Service Description:

Financial assistance based on available funding from the foundation * assists people who require support to pay for eyeglasses, assistive devices, dentures and dental work
Any additional funding that might be available requires assessment and a follow up application to the Foundation.

Applications can be picked up at one of the following locations:
- District of Muskoka Administration Building, 70 Pine St, Bracebridge, ON
- Huntsville Employment Resource Centre, 110 Main Street E Huntsville, ON
- Ontario Works Satellite, 1 King William St Huntsville, ON
- Ontario Works Satellite, 141 Main St Gravenhurst, ON
The application, including an estimate of the funds needed, must be filled out completely and mailed to the PO Box noted.
