Service Description:

Community-based program that works with a community service provider to navigate and establish individualized supports to either find a job, start a business or career advancement
Services include:- job preparation
- on-the-job supports
- adaptive technology
- assistive devices and training
- assistance to develop a business plan (if starting a business)
- access to a service provider that specializes in self-employment

People who have a substantial physical or mental disability that is expected to last a year or more, and has difficulty to find or keep a job
