Ontario is taking the next steps in its plan to better
integrate the province’s health care system to help end hallway health care. To
support this, we are aligning with four other regions in Central Ontario so
that we can work more cohesively to stay on top of health care needs while also
working to help patients get the care they need locally, anywhere in the region
or across Ontario. 



 



You will continue to have access to the care you need and
the health care providers you have built relationships with.



 



Home care continues as before and there are no changes to
long-term care home placement.



 



Our patient services and contacts will remain unchanged
until further notice.
