Position:  Clinical Documentation Improvement Specialist #1647

Region:  Barrie & Area

Description:  The Clinical Documentation Improvement Specialist (CDI) facilitates improvement in the overall quality, completeness and accuracy of medical documentation. This individual will perform concurrent and/or retrospective medical record reviews. This position interacts with Physicians, Clinical staff, and Coding Specialists to ensure accuracy and completeness of clinical documentation which supports patient care, funding, research and hospital utilization. This individual will provide education, develop tools and support the Physician query process.

Building rapport/relationships
•Foster relationships between various disciplines
•Liaison between clinical and business side of healthcare
•Facilitate extensive day-to-day interaction with physicians and coding specialists

Analysis
•Target clinical groups and chart selection
•Chart review and analysis (review record for complications/comorbidities that were monitored, evaluated or treated; search for clinical evidence to support presence of a diagnosis versus coding of a symptom)
•Send queries and follow up (abnormal test results not addressed; Medication ordered without supporting diagnosis; Conflicting documentation between providers) *Query is verbal or written communication between a coder and physician for increased specificity and clarification of documentation and accurate code assignment.
•Summary of results (Identify trends in data to determine opportunities for education)
•Present findings to enable timely, accurate and complete documentation of clinical information used for measuring and reporting physician and facility outcomes.

Qualifications

Education:
•Graduate from an approved Health Information Management Program; or
•Degree in Health Information Management; I and II require certification with CHIMA and in good standing ; or
•Bachelor of Science in Nursing Degree

Experience:
•Minimum of two years’ experience in nursing or other clinical area, Inpatient coding, process improvement
•Utilization review/case management in an acute care facility
•Demonstrated knowledge of Inpatient coding utilizing Canadian Coding Standards and ICD10/CCI
•Working knowledge of Health Based Allocation Model Inpatient Grouping methodology (HIG) and Health System Funding Reform (HSFR)

Salary:  $37.27-$43.68/Hour

Terms of Employment:  Full-Time
