Service Description: Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible * referral to other community services

Provides comprehensive community health, support and in-home services by nurses, personal support workers and volunteers
 
Caregiver Support Circle - Peer support groups, education, and discussion for caregivers * referrals to community services
 
Community Nursing - General nursing care including visiting and shift nursing
 
Cool-Aid Program - Clients receive assistance in completing medical information in a provided template, so it is accessible for emergency personnel in the event of an emergency
 
Falls Prevention and Group Exercise Classes for Seniors - Group exercise and falls prevention classes for older adults and people with mobility issues and chronic conditions * aims to improve strength and balance to prevent injury and falls
 
Home At Last Program - Assists older adults returning home from a hospital stay
 
Supportive Care Program - Hospice Visiting Service - Hospice volunteers provide companionship, respite care, and emotional support to people and their family/caregivers dealing with a life-limiting or life-threatening illness * bereavement support for anyone who has experienced the death of someone close * services can be provided at home, or in hospitals and long-term care homes * palliative care education
 
Supports For Daily Living - Care for older adults with chronic illness or cognitive impairment who require assistance with independent living
